Physio Appointments for Glendowie College Students

Habit Health Kohi Physio is onsite Tuesdays and Fridays from 11:00am to 3:00pm

• A consent form is required to be signed by the parent/caregiver prior to the appointment.
• Procedures for booking:
  • Parents/caregivers/students to phone Habit Health Kohi directly on (09) 528 6400 to make an appointment at school. NB there is no longer a booking sheet outside Nurse’s Room.
  • A consent form will be emailed to parent/caregiver to sign and return via email.
  • The clinic will book the appointment at Glendowie College.
  • Appointment reminders will go out to the students on the day of treatment via the school
• COST: The charge is $20.00 for ACC treatment. This will be invoiced and emailed directly to parent/caregiver for payment.
• 24-hour Cancellation Policy - If the student DNA’s (Did Not Attend) a charge of $20.00 will be invoiced.
